The following forms are available for reporting quarterly attendance and progress reports.

 

Attendance Report - This form is available as either a pdf document or a Microsoft Excel file.

 

Progress Report- This form is also available as either a pdf document or a Microsoft Excel file.

 

The pdf documents may be printed out and completed by hand.  See parent's manual for instructions. 

 

The Microsoft Excel files are for those with Microsoft’s Excel program on their computer. There are several advantages to using the excel program rather than completing them by hand.  The final reports are easier to read than a handwritten report and the program will minimize the time it takes to complete your reports. The majority of information on the Attendance and Progress reports need to be completed just once at the beginning of the school year.  Keep a copy on your computer and just add the additional data for each quarter.

Attendance Report:

If you use the "calculated" version (see the blue tab on the bottom of the worksheet), when you put an "x" on the attendance report on the date that your child “attended” school, the program will automatically calculate the total number of days for you. Also, based on the grade level you enter at the top of the form, the program will automatically calculate how many hours that equates to.  Each quarter builds upon the next - with excel keeping a cumulative total of the days and hours. 

If you choose to use the manual version (look for the purple tab at the bottom of worksheet), the days and hours will not be calculated for you.

Progress Report

Simply change the marking period (quarter), date and add new grades as the year progresses on the progress report.

Keep a copy of both the attendance and progress reports on your computer so it will be easier to find and update your reports when you need them.  This eliminates the need to hunt down your paper copy that perhaps you may have misplaced :).

 

The Attendance Report and the Progress Report must be mailed to the school, 568 SE Port St. Lucie Blvd., Port St. Lucie, FL, 34984, with your grade advisor's name on the front bottom left corner of the envelope. Emailed copies are not accepted.
